2008 Pacific/USA Swimming Year-Round Athlete Registration Form 2008 Pacific/USA Swimming Seasonal Athlete Registration Form |
These forms are for use by athletes to register or to renew their registration. Note that there are separate forms for year-round registration and seasonal registration. Please use the appropriate form for your situation.
The 2008 year-round form is valid from Sept. 1, 2007 through Dec. 31, 2008. Seasonal registration for 2008 is valid only from Sept. 10, 2007 through Feb. 6, 2008, (season 2) or from April 5, 2008 to September 1, 2008 (season 1).
The forms may be downloaded and copied, but please cut the page in half to submit a single registration because there are two forms to a page.
Due to increases in registration and membership fees charged in recent years by USA Swimming, Pacific Swimming's board of directors has increased the Pacific Swimming registration/membership fees for 2008. The fees for 2008 will be $55 for an annual athlete, $33 for a seasonal athlete, and $45 for an non-athlete member. The club membership fee for 2008 is unchanged at $120.
Athletes affiliated with a club should look to their club for assistance in the registration process, as many clubs choose to register their swimmers electronically.

2008 Pacific/USA Swimming Non-athlete Membership Form

For use by coaches, officials, meet directors, and other non-athletes to join or to renew their membership.
(Note to coaches: follow this link for a list of organizations providing approved courses in first aid, CPR, and safety training that you can use to obtain or renew your coach membership in Pacific/USA Swimming. Also, here is a list of local safety training providers.)
(Note also that meet marshals are no longer required to be non-athlete members of USA Swimming.)

NTV Form A
|
This form is to be used to request observation for verification purposes at YMCA, high school, collegiate or Masters meets that are not district, conference, league, or national championships.

NTV Form B
|
This form is to be used to request authorization for observation of high school, YMCA, collegiate, or Masters meets for verification purposes. If high school, the meet must be league or sectional championship; if collegiate, the meet must be in the qualifying progression for the season culminating championship.

To be used by eligible members of Pacific Swimming traveling to the 2008 U.S. Aquatic Sports convention to request reimbursement of designated travel expenses. (Note: Treasurer Fred Vogelgesang will cut a check for the early-bird registration if a convention attendee sends the filled out registration form to him. He will then attach a check and mail it if received before August 31. After that date, it should be mailed direct.)
