2009 Pacific/USA Swimming Seasonal Athlete Registration Form

These forms are for use by athletes to register or to renew their registration. Note that there are separate forms for year-round registration and seasonal registration. Please use the appropriate form for your situation.

The 2009 year-round registration form is valid through Dec. 31, 2009. Seasonal registration is available for summer 2009 and is valid from April 11, 2009 to September 7, 2009.

The forms may be downloaded and copied, but please cut the page in half to submit a single registration because there are two forms to a page.

Athletes affiliated with a club should look to their club for assistance in the registration process, as many clubs choose to register their swimmers electronically.

2009 Pacific/USA Swimming Non-athlete Membership Form For use by coaches, officials, meet directors, and other non-athletes to join or to renew their membership.

(Note to coaches: follow this link for a list of organizations providing approved courses in first aid, CPR, and safety training that you can use to obtain the certifications necessary to obtain or renew your coach membership in Pacific/USA Swimming. Also, here is a list of local safety training providers.)

(Note also that meet marshals are no longer required to be non-athlete members of USA Swimming.)

2009 Pacific/USA Swimming Club Application Form For use by prospective clubs to join or by current member clubs to renew their membership. Clubs may join as year-round or seasonal; see the club membership application form for detail on dates.

Please note that if you are starting a new club, there are a number of other forms you need to complete and a number of other issues you need to consider. You can find links to these forms and more information about the requirements for starting a club on the Starting a New Club page on the USA Swimming web site.

In addition to competing a club membership application, you will need a coach who is a member of USA Swimming and holds valid CPR, first aid, and safety training certifications, and you will need registered swimmers. Every coach (paid or volunteer) who is on the deck at a club workout or coaching at a meet must be a current coach member of USA Swimming. Your club must renew its membership each calendar year, and as part of this renewal process, your coaches must submit proof of current CPR, first aid, and safety training certifications.

Meet Sanction and Meet Management
Form Description
Applying for a Meet Sanction (Instructions)
Sanction Application Form
To be used to apply for a meet sanction within Pacific Swimming.
Meet Approval Report To be used to apply for approval from Pacific Swimming for a non-Pacific Swimming meet.
Safety Guidelines and Warm-up Procedures Must be posted within the meet venue at a meet sanctioned by Pacific Swimming.
Swim Meet Financial Form (PDF version)

Financial report to be submitted to Pacific Swimming after a sanctioned meet (save time and use the Excel spreadsheet version).
Report of Final Day No-Show To be submitted when a swimmer is a no-show for finals at a T&F meet on the last day the swimmer is entered in an individual event in the meet, and no other individual event penalty is appropriate. (Reference: Rules and Regulations section 3.B.5.b.4.ii)
Dual Meet Referee Report To be submitted by the meet referee after a sanctioned dual/tri/quad meet.
Sending Results Instructions on how to send your meet results to Pacific Swimming after your meet
Meet Officials Report Form 1 To be used by meet referees to submit meet data and evaluations to Pacific Officials Chairman

Insurance
Form Description
Report of Occurrence Form USA Swimming accident report form, to be completed for any accident at a meet, practice, or other team function.
Additional Insured Endorsement This form will help you obtain an additional insured endorsement if your club's contract with a facility requires that they be named as an additional insured on your club's insurance policy. Follow this link for more information.

Teaching Racing Starts
Form Description

Times Verification for non-USA Swimming Meets
Form Description
National Times Verification/Observed Meets Information on National Times Verification and Observed Meets.
NTV Form A This form is to be used to request observation for verification purposes at YMCA, high school, collegiate or Masters meets that are not district, conference, league, or national championships.
NTV Form B This form is to be used to request authorization for observation of high school, YMCA, collegiate, or Masters meets for verification purposes. If high school, the meet must be league or sectional championship; if collegiate, the meet must be in the qualifying progression for the season culminating championship.

 

Travel Assistance
Form Description
Application for Pacific Swimming Travel Assistance (2009 meets) To be used by swimmers or their clubs to apply for the designated travel assistance for national meets taking place in 2009.

Income/Disbursement
Form Description
Spending Approval Request form This form is used as a template for a Board of Directors motion to seek the approval to spend a budget line item with an asterisk. All budget line items with an asterisk must receive BOD approval prior to spending the item. Please be aware that any contract, agreements, purchase orders or similar obligations equal to or greater than $5,000 must have two signatures and a copy must be reviewed by the Treasurer prior to signing.
Request for Reimbursement/Check Request Form For requesting reimbursement or disbursement from Pacific Swimming's treasurer.
Income Form For submitting Pacific Swimming income to Pacific's treasurer.

2010 Bid Package and Meet Criteria for Major Pacific Meets
Document Description
2010 Bid Package To be used by clubs wishing to bid to host a meet on Pacific Swimming's 2010 major meet calendar.
Age Group Meet Criteria Specifies facility, time standards, personnel, format, awards, and other criteria which clubs must meet to host major age group meets on the annual Pacific major meet calendar.
Senior Meet Criteria Specifies facility, time standards, personnel, format, awards, and other criteria which clubs must meet to host senior meets on the annual Pacific major meet calendar.

Pacific Swimming Pool Certification Form Used when required by Pacific Swimming to certify the dimensions of a competition pool.
USA Swimming Pool Certification Form Used when required by USA Swimming to certify the dimensions of a competition pool.
