2012 Pacific/USA Swimming Year-Round Athlete Registration Application

2012 Pacific/USA Swimming Summer Season Athlete Registration Application



These forms are for use by athletes to register, to renew their registration, and to transfer their registration.

2012 year-round registration is valid through Dec. 31, 2012. The fee for 2012 year-round registration is $65.

The 2012 summer seasonal registration form is valid from April 7 through September 3, 2012. The fee for 2012 summer season registration is $45.

The registration forms may be downloaded and copied, but please trim the page as requested before submitting it.

Athletes affiliated with a club should look to their club for assistance in the registration process, as many clubs choose to register their swimmers electronically.

Transfer Form For use by athlete members when transferring their affiliation from one club to another within Pacific or when transferring into Pacific from another LSC.

2012 Pacific/USA Swimming Non-athlete Registration Application For use by coaches, officials, meet directors, and other non-athletes to join or to renew their membership. The fee for 2012 non-athlete membership is $60.

Coaches, follow this link for a list of organizations providing approved courses in first aid, CPR, and safety training that you can use to obtain the certifications necessary to obtain or renew your coach membership in Pacific/USA Swimming. Also, here is a list of local safety training providers.

It is necessary to complete a USA Swimming background check in order to become a non-athlete member of Pacific/USA Swimming. The following link will take you to a page on the USA Swimming website with details about the background check requirements and a link to initiate the background check process.

2012 Pacific/USA Swimming Club Application Form

The fee for 2012 club membership is $225 for year-round clubs and $125 for summer season clubs.

For use by prospective clubs to join or by current member clubs to renew their membership. Clubs may join as year-round or seasonal; summer season club registration is valid from April 7 to September 3, 2012.. Please note that the application form must be filled out completely.

Please note that if you are starting a new club, there are a number of other forms you need to complete and a number of other issues you need to consider. You can find links to these forms and more information about the requirements for starting a club on the Starting a New Club page on the USA Swimming web site.

In addition to competing a club membership application, you will need a coach who is a member of USA Swimming, has passed USA Swimming's background check, and holds valid CPR, first aid, and safety training certifications. In addition, you will need registered swimmers. Every coach (paid or volunteer) who is on the deck at a club workout or coaching at a meet must be a current coach member of USA Swimming. Your club must renew its membership each calendar year, and as part of this renewal process, your coaches must submit proof of current CPR, first aid, and safety training certifications.

Athlete and Coach Travel Assistance
Form Description
Application for Pacific Swimming Travel Assistance -- 2012 meets

(updated as of February 29, 2012 -- prior versions are obsolete)

This program provides an allowance for travel to national championship-level meets for athletes and coaches. The travel allowance for athletes includes an amount for airfare plus a stipend, both of which are determined prior to the meet by the Board of Directors. This amount is publicized on the Pacific website.

The person or club requesting reimbursement is required to include receipts with the request for travel assistance. The receipts can be for airfare, hotel, food or other related travel expenses, up to the amount of the allowance for the meet passed by the Board of Directors. See the summary on the back of the form for details on reimbursable expenses. The request for reimbursement be received by Pacific's Registration office within 60 days from the end of the meet.

Application for Supplemental Travel Assistance -- 2012 meets

The supplemental travel assistance program provides reimbursement for athletes' travel to high-level senior meets in addition to the reimbursement described above for national championship meets. The fund provides one $200 reimbursement per year. See the form to the left for more detail on eligible meets and on qualification requirements.

Travel Reimbursement Summary Provides specifics on which travel expenses are reimbursable under the Travel Assistance and Supplemental Travel Assistance programs for athletes.

Application for Coach Travel Assistance
To be used by coaches to apply for travel assistance for national championship meets. The amount of assistance for coaches is the amount approved for airfare for athletes traveling to the same meet.

Travel Policy Guidelines and Recommendations Includes a generic template travel policy for clubs approved by USA Swimming Board of Directors. All clubs seeking travel assistance through Pacific Swimming must prepare a written travel policy and must have a hard copy of their travel policy on file with Pacific Swimming Office-Membership, 1374 Lupine Court, Concord, CA. 94521, office-membership@pacswim.org. Any club that has not completed this task with Pacific Swimming will not qualify for travel reimbursement offered through Pacific Swimming?s Disbursement Policy.

Insurance
Form Description
Reporting an Occurrence The USA Swimming Report of Occurence form is to be completed for any accident at a meet, practice, or other team function. It can now be completed online at the USA Swimming website. Follow this link for a hard copy of the Report of Occurrence form which you can use to gather the information you need to complete the online form.

After filing an online Report of Occurrence, you will receive an automated acknowledgement email. Please forward a copy of this email to the Pacific office.

Additional Insured Endorsement This form will help you obtain an additional insured endorsement if your club's contract with a facility requires that they be named as an additional insured on your club's insurance policy. Follow this link for more information on requesting the certificate.
Insurance Coverage USA Swimming offers, as a benefit of membership, insurance coverage to its members and member clubs to include General Liability and Excess Accident Medical coverage. Times Verification for non-USA Swimming Swim Meets
Form Description
Requirements for Observed Swims Information on procedures for observed meets.
Request for Observation -- Form A This form is to be used to request observation for time verification purposes for high school, collegiate, YMCA, or Masters meets that are not district, conference, league, or national championships.
Request for Observation -- Form B This form is to be used to request authorization for observation of high school, YMCA, collegiate, or Masters meets for verification purposes. If high school, the meet must be a league or sectional championship; if collegiate, the meet must be in the qualifying progression for the season culminating championship.
Relay Leadoff Times Request Form This form is to be used to log relay leadoff splits. A copy must be submitted to the Pacific Swimming Office after the meet.
DQ Log This form is to be used to keep a log of all USA Swimming stroke and turn violations. A copy must be submitted to the Pacific Swimming Office after the meet.

Income/Disbursement
Form Description
Spending Approval Request form This form is used as a template for a Board of Directors motion to seek the approval to spend a budget line item with an asterisk. All budget line items with an asterisk must receive BOD approval prior to spending the item. Please be aware that any contract, agreements, purchase orders or similar obligations equal to or greater than $5,000 must have two signatures and a copy must be reviewed by the Treasurer prior to signing.

Check Request Form
For requesting reimbursement from Pacific for items purchased, or payment from Pacific for services provided. (Note that tax identification number and current address must be provided before any payment for services provided to Pacific will be made.)

W-9 Form Must be submitted with Request for Reimbursement form when payment for services provided to Pacific will be made directly to a vendor.
